With the end of 2018 already in sight, we have created an electronic membership renewal form for Provider Members using Google Forms. If you would prefer a paper version, please contact us.

Our membership, and therefore our impact, has grown steadily in 2018, with the addition of nine new Provider Members, four new Affiliate Members, and a new permanent member of staff. While this has been a challenging year for the program, the resourcefulness and dedication of our members has been nothing short of impressive. We look forward to facing the future as a strong and unified Association. As always, we encourage you to provide feedback on how CDPAANYS can better serve your organization’s mission.
